If the candidate for water baptism is a minor, this form must be completed by a parent or guardian.
The candidate attests to the following:
• I have accepted Jesus Christ as my personal Savior.
• I desire to be obedient to the Holy Spirit (Matthew 28:19).
• I believe that immersion is the correct method of baptism as this ordinance represents the burial and the resurrection with our Lord Jesus Christ (Romans 6:4).
• I understand that this baptism is not equivalent to joining this church and does not obligate me to become a member.
